What happens when the health and social services sectors come together?

Oct. 25, 2021 / By L. Gordon Moore, MD

This is the question behind a publication from Dr. Will Bleser and colleagues at the Duke Margolis Center for Health Policy.

Anyone doing or contemplating work similar to this has likely considered the many challenges:

  • What data do we need and how do we track the information?
  • Who is a good partner and how can we share information?
  • How do we collaborate with partners outside of the usual health care delivery system?

In this podcast, Dr. Bleser describes research findings on who is doing this work, pilots to watch, approaches to standardization of tools and information sharing. He also talks about ways to set up for success, especially in collaboration with social service agencies and community-based organizations who have been doing this work for a long time but lack the resources of the health care delivery system.

Dr. Bleser’s work is worth following as our nation explores ways to improve outcomes through a collaborative and coordinated approach to non-medical factors that impact outcomes.

Dr. Gordon Moore is senior medical director, Clinical Strategy and Value-based Care for 3M Health Information Systems.
